3420 - HEALTH INSURANCE BENEFIT

The Board of Education shall provide health insurance to all eligible staff members in accordance with the plan described in the District's Health Insurance Plan.

Those employees who have other coverage may waive the District's health insurance coverage. Those staff members who waive the insurance will be paid an additional monthly compensation with the understanding that this additional compensation is subject to FICA, FUTA, and Federal, State, and local income tax. The waiver must be in writing and be accompanied by evidence of other health insurance coverage. Monthly compensation of $500 will be paid if the employee qualified for a family plan and $300 will be paid if the employee qualified for a single plan.

Staff members may either waive insurance coverage or revoke a previous waiver on an annual basis and in accordance with procedure described in the plan.

The monthly compensation will be prorated for employees less than 100%.

THIS POLICY SHOULD BE ADOPTED ONLY AS PART OF A FLEXIBLE BENEFIT PLAN, ESTABLISHED BY THE DISTRICT, THAT MEETS ALL APPLICABLE REQUIREMENTS OF SECTION 125 OF THE IRS CODE 1986, AS AMENDED, AND RELATED IRS REGULATIONS.
