5300 - FOOD ALLERGY
The District recognizes that food allergies, in some instances, may be severe and even life threatening. The District has adopted this policy to create a framework for accommodating students with food allergies and to reduce the likelihood of severe allergic reactions of students with known food allergies while at school.
Parent/Student Responsibility:
| A. | Parents of students, or students age eighteen (18) or older, with allergies must notify the District of such allergies at the beginning of each school year. Such notice may be provided on the District’s 5341F1 form completed by the parents, or by other acceptable means such as written notice to the school nurse or building principal. | ||
| B. | Parents of students with life threatening allergies must: |
| 1. | Provide the District with emergency medications; | |||
| 2. | Execute an appropriate Medical Authorization Form (see 5341 F1), a School Administration of Drugs Prescribed by Physician Form (see 5330 F1), and/or Food Allergy Action Plan as required; and | |||
| 3. | Cooperate with the District to formulate a FOOD ALLERGY ACTION PLAN (8515F1) in accordance with AG 8515 and described more fully below. |
| C. | Parents are responsible to educate their student about managing his/her allergy at school, including, but not limited to, identifying "safe foods," by reviewing the weekly lunch menu together, and discussing the vigilance required to self monitor food products sold at athletic events or special student sales, foods brought for potlucks or classroom celebrations, or foods served on school field trips. |
District Responsibility:
| A. | The District must cooperate in the development of a FOOD ALLERGY ACTION PLAN (8515 F1) for students with life threatening allergies in accordance with AG 8515. | ||
| The FOOD ALLERGY ACTION PLAN (8515 F1) must address (1) what actions will be taken to avoid exposure at school, and (2) what actions will be taken in the event of exposure. The Plan shall be developed through consultation between the school nurse, the student’s parents, and the student’s physician or allergist. Once created, this Plan should be reviewed and updated annually by the school nurse, the student’s parents, and the student’s physician or allergist. | |||
| B. | The District will share the FOOD ALLERGY ACTION PLAN (8515 F1) developed with appropriate school staff. | ||
| C. | With the consent of the student’s parents, a FOOD ALLERGY ACTION PLAN (8515 F1) may provide a mechanism for the District to provide notice to the student’s classmates and/or a student’s classmates’ parents of a life threatening food allergy in the classroom. |
