3438 - SABBATICAL LEAVE
Any teacher who has completed five (5) years of service in the Perry Local School District may be entitled to take a leave of absence for one or two semesters subject to the following restrictions:
| A. | Application submitted by March 1st of the school year prior to the leave. | |
| B. | A plan of study in education approved by the Superintendent. | |
| C. | The applicant will notify the Perry Local Schools by December 15 and/or April 15 during his/her leave of his/her intent for the following semester. | |
| D. | No more than two (2) people may be on leave at one time. | |
| E. | A person on leave does not receive service credits. |
Rights While On All Leaves
The staff member on leave shall be entitled to request and receive the right to continue to be covered by insurance for hospitalization, surgical benefits and major medical, dental insurance, and life insurance, so long as the employee reimburses the Board monthly the cost of said insurance at the Board rate.
