3420 - INSURANCE BENEFITS
The Governing Board shall obtain health insurance for all full-time staff members in accordance with the plan described in the Center's Health Insurance Plan.
Perry-Hocking Educational Service Center employees, both certified and non-certified, will qualify for the Governing Board share of paid fringe benefits under the following policy.
| A. | Employees under contract for twenty (20) hours or more per week qualify for 100% of the Governing Board share of fringe benefits. | ||
| B. | Employees with no contract, a consultant contract, working as a substitute, or under contract for nineteen (19) hours or less per week will not qualify for Governing Board paid benefits. Employees not qualifying for the Governing Board paid benefits may purchase benefits subject to approval by the insurance carrier. Premiums must be paid in full one (1) month in advance of due date. | ||
| C. | A day is equal to seven (7) hours for non-hourly employees. |
GROUP HEALTH INSURANCE
The Board realizes the concern of its employees for the availability of those protective and personally advantageous benefits beyond an individual's basic salary. It is the Board's desire to make available, or provide within the limits of the law and sound fiscal management, those which are beneficial to the employee and the District.
The Board shall provide group health insurance for employees of the Service Center District eligible to participate in accordance with statute and within the guidelines established by the insurance carrier and the Board of Education. Participation in this plan is optional.
Open enrollment will be held during the month of June each year and/or thirty (30) days from the date of initial employment. All applications, other than those within thirty (30) days of initial employment, will be subject to approval by the insurance carrier.
The Board shall pay the amount on each policy as established annually by resolution of the Board. All other costs will be paid by the employee.
GROUP LIFE INSURANCE
The Governing Board will make available to its employees a group life insurance program in the amount of $20,000 coverage for employee. The premium for the coverage in this program shall be paid by the Board for all employees who meet the requirements above and who request said coverage.
GROUP DENTAL INSURANCE
The Governing Board shall provide group dental insurance for the employees of the Service Center District eligible to participate in accordance with statute. Participation in this plan is optional.

GROUP VISION INSURANCE
The Governing Board shall provide group vision insurance for the employees of the Service Center District eligible to participate in accordance with statute. Participation in this plan is optional.
The Board shall pay the amount on each policy as established by resolution by the Board.
Staff members may either waive insurance coverage or revoke a previous waiver and thereby seek reinstatement on an annual basis in accordance with the procedure described in the plan.
Medical Deductible Reimbursement
| A. | Participant submits claim as submitted to Administrator at HR Butler. | ||
| B. | Claim is audited for validation. |
| 1. | Claim will either be applied to their first $500.00 deductible that must be met OR | |||
| 2. | Claim will apply to their last $1,500 reimbursable portion of their deductible. |
| C. | Claim will be entered into software system for payment processing. | ||
| D. | Payment processing will occur every week on Tuesday and Friday. Checks will be cut directly from the Perry-Hocking ESC bank account. | ||
| E. | Payment register will be emailed to ESC Treasurer and Assistant the day that payment has been processed for a Perry-Hocking ESC participant. This information is also available in real-time on myRSC.com. | ||
| F. | Participant's reimbursement will be directly deposited into their personal account or mailed to their home address. |
