6321 - ADDING NEW VENDORS

The District will authorize vendors who meet the following conditions:

	A.	O.R.C. Section 9.24 prohibits any state agency or political subdivision from awarding a contract for goods, services or construction to any person against whom a find for recovery has been issued by the Auditor of State, if that finding is unresolved. The District only contracts with vendors who have no unresolved Findings of Recovery.

	B.	All vendors must provide a W-9 Form.

	C.	All vendors must complete a New Hire Reporting Form.

The District will maintain the following hard copy documentation on every authorized vendor:

	A.	Finding for Recovery documentation found on the Auditor of State’s website

	B.	W-9 Form

	C.	New Hire Reporting Form

The District will issue a 1099 to all sole proprietors or those using their social security number for tax identification, who have been paid $600 or more.
