8320.01 - PERSONNEL RECORDS
At the time of initial employment each employee shall file in the central office, prior to the first day of employment, the following:
| A. | a complete employment history; | ||
| B. | record of education (including official transcripts, if required); | ||
| C. | military record and record of discharge (if applicable). |
A personnel file on all employees shall be maintained in the central office. This shall be considered a confidential file and the file of recorded information of employees maintained by the Corporation for the School Board.
The individual employee, upon reasonable request, shall have access to his/her personnel file at the central office. Request of employees to have access to their personnel files shall be handled by the Superintendent.
Groups and individuals authorized to use personnel files of employees shall be limited to the Board, Superintendent, management personnel directly related to those employees or who are responsible in directing the services of employees, other individuals authorized by the Superintendent and appropriate clerical staff of the central office with which the above stated management personnel have direct responsibilities.
All materials in the personnel file of employees shall include, but not be limited to:
| A. | correct name, address, and telephone number; | ||
| B. | a complete employment history; | ||
| C. | record of education (including official transcripts, if requested); | ||
| D. | military record and record of discharge (if applicable); | ||
| E. | copy or copies of appropriate certificates (if applicable); | ||
| F. | any data required concerning health and/or medical examination; | ||
| G. | record of assignments and/or employment; | ||
| H. | record of leave accumulation and use; | ||
| I. | evaluation of performance; | ||
| J. | copies of anecdotal records; | ||
| K. | letters of recommendation and other related information used for initial employment may be retained. If letters of recommendation are filed on the personnel file of an employee they shall not be subject to the provisions herein stated. |
