4421.03 - BENEFITS WHILE UNDER WORKER’S COMPENSATION

When an employee has been accepted for benefits under the Worker's Compensation law for any reason(s) included in R.C. 4123, s/he shall be placed on a leave of absence for a period not more than two (2) years after the date of the injury or medical diagnosis of the occupational disease in compliance with R.C. 3319.13. The Board will pay for all Board-approved premiums for all or any part of the insurance plans available to all employees for the two (2) year duration of the leave of absence.

At the end of the two (2) year period the employee may request that the leave of absence be renewed by the Board. If the leave of absence is renewed, the employee may retain any part or all of the Board-approved insurance benefits by making the payment of the premium.

It shall be the responsibility of the employee on leave to make such payment by check, payable to the Board, unless otherwise advised, and submitted to the Treasurer's Office by the twenty-fifth (25th) day of each month preceding the month for which the premium is due.
