5341 - EMERGENCY MEDICAL TREATMENT AUTHORIZATION
The District shall maintain and keep available an emergency medical treatment authorization form for each student in attendance.
At the beginning of each school year a new form with a request for revision and a new signature shall be distributed to parents.
The Superintendent shall develop and maintain administrative guidelines for the content and administration of these forms which shall:
|A.||assure that authorization for emergency medical treatment for the student is granted by the signature of one of his/her parents on the form;|
|B.||allow for special health-related instructions to be recorded on the form;|
|C.||authorize a physician or medical emergency unit to be called in case of an emergency;|
|D.||assure the availability of copies of these forms to proper staff persons in charge of school and/or school related activities.|
In the event that emergency medical treatment for a student is necessary, the District shall adhere to the instructions on the emergency medical treatment authorization form.
Under no circumstances shall any school personnel abide by any Do Not Resuscitate (DNR) agreement that may exist for a student, unless ordered to do so by a court of law.
