6700 - PERSONAL FINANCIAL STATEMENTS

The Superintendent and Treasurer shall submit annually to the Ohio Ethics Commission a financial statement covering himself/herself and his/her immediate family which shall contain the following:

 A.Every source of income and income of others for his/her use or benefit which exceeds $500, including honorariums, and a description of the nature of services for which income was received for the preceding calendar year. Sources such as attorney's clients, physician's patients, and others stipulated by statute need not be identified.

 B.The name of each Ohio corporation, or other corporation, trust, business trust, partnership, or association certificated to do business in Ohio, in which had an investment valued at more than $1000 as of December 31st or the date of disposition during the preceding year, whichever is earlier. The statement shall describe the nature of each investment.

 C.The name of each Ohio corporation in which s/he holds an office or has a fiduciary relationship and a description of the office or relationship unless exempted by statute or regulation.

 D.All fee simple and lease-hold interests in which s/he holds legal title to or a beneficial interest in real property located in Ohio, excluding the personal residences and property used primarily for personal recreation.

 E.The names of all persons residing in or transacting business in Ohio to whom s/he owes, in his/her name or in the name of another person, more than $1000 unless exempted by statute or regulation.

 F.The names of all persons residing in or transacting business in Ohio who owes him/her more than $1000, unless exempted by statute or regulation.

 G.The source of each gift of more than $500 received during the preceding calendar year by the Superintendent or Treasurer in his/her own name or in the name of any other person, for his/her use or benefit, unless exempted by statute or regulation.

The financial statement may be submitted by mail or in person and must be accompanied by the required filing fee of five dollars ($5.00.)

The Ohio Ethics Commission shall inform the Superintendent or Treasurer of any potential conflict of interest it determines exists from its inspection of his/her financial statement and make those portions of the statement which indicate potential conflict of interest available for public inspection. All other portions of the financial statement shall be kept confidential and unavailable for public inspection, except when inspection is required by law. The financial statements of the Superintendent and Treasurer shall be available to the District's auditors when conducting required audits.
