3139.01 - DOCUMENTATION OF DISCIPLINARY ACTION
It is the policy of the Avondale School District that all District employee behavior resulting in disciplinary action of any type is to be both clearly documented in writing and made known to the supervisor(s) of the disciplined employee. To that end, the Board of Education directs that, effective immediately:
| A. | All documentation of staff discipline including, without limitation, written reprimands shall be placed in the District’s personnel file for the disciplined employee in an open and unsealed condition. | ||
| B. | The line supervisor(s) of each disciplined employee shall be clearly informed of the conduct which led to the employee’s discipline and of the discipline imposed upon the employee. | ||
| C. | The Superintendent or his/her designee shall immediately review the personnel file of every district employee to ascertain the existence of any sealed documents or envelopes in each such file, and shall open and review the contents of every such sealed document and envelope. The Superintendent shall, within thirty (30) days after the adoption of this policy: |
| 1. | Advise the Board of Education in writing of the number of sealed documents or envelopes found in the personnel files of District employees; | |||
| 2. | Summarize (1) the contents of each such document or envelope and to the extent that such contents documented or referenced discipline, (ii) the discipline imposed and the conduct which led to such discipline. |
| D. | All disciplinary action documented in a District teaching employee’s personnel file, and the conduct which led to such action, shall, to the fullest extent permitted by law, be disclosed to the Board prior to any granting of tenure to the employee, and in sufficient time for the Board to evaluate and consider such conduct and disciplinary action before the employee is granted tenure. |
